Job Title: Social Worker – Young Adults Transitions Team

Type: Temporary / Full Time
We are seeking an experienced Social Worker to join a dedicated Young Adults Transitions Team based in Herefordshire. This is an excellent opportunity to support young people aged 18–25 as they transition from Children’s Services into Adult Social Care, helping them achieve positive outcomes, greater independence, and improved wellbeing.
Working within a strengths-based framework, you will undertake assessments, develop care and support plans, and work collaboratively with young adults, families, and partner agencies to ensure safe and effective transitions into adult services.

What You Will Do
Transitions Assessments: Undertake strengths-based assessments to identify needs, outcomes, and appropriate support arrangements for young adults transitioning into Adult Social Care.
Care Planning: Develop and review person-centred care and support plans that promote wellbeing, independence, and positive life outcomes.
Safeguarding Practice: Undertake safeguarding activities, including Section 42 enquiries, where required.
Mental Capacity Assessments: Complete Mental Capacity Act assessments and contribute to best-interest decision-making processes.
Continuing Healthcare (CHC): Participate in Continuing Healthcare assessments and multi-agency decision-making processes where appropriate.
Multi-Agency Working: Work collaboratively with Children’s Services, health partners, education providers, families, carers, and other professionals to support effective transitions.
Court Work and Reporting: Contribute to court work and prepare professional assessments, reports, and supporting documentation where required.
Case Management: Manage a varied caseload while maintaining accurate records and ensuring compliance with statutory responsibilities.
What We Are Looking For
- Qualified Social Worker with current Social Work England registration.
- Experience within Adult Social Care services.
- Knowledge of the Care Act 2014 and strengths-based practice.
- Experience undertaking safeguarding work, including Section 42 enquiries.
- Understanding of the Mental Capacity Act and best-interest decision-making.
- Experience of assessment, care planning, and multi-agency working.
- Excellent communication, analytical, and report-writing skills.
- Ability to build effective relationships with young adults, families, and professionals.
