Job Description
Job Title: Social Worker – Urgent & Emergency Care Team
Specialism: Adult Social Care
Location: Nottinghamshire, UK
Salary: Umbrella £27.40 Per Hour
Type: Temporary / Full Time
We are seeking an experienced Social Worker to join the Urgent and Emergency Care service in Nottinghamshire.
This is an excellent opportunity for a qualified Social Worker to support adults with complex needs through assessment, support planning, safeguarding and discharge planning. The successful candidate will work within a strength-based and person-centered approach, promoting independence, wellbeing and positive outcomes.
What you will do
Assessment & Support Planning
- Manage a complex caseload of adults requiring social care support.
- Complete assessments, reviews and support plans, including health and social care assessments.
- Identify opportunities for reablement and preventative support.
- Provide advice and guidance to individuals, careers and professionals.
Safeguarding & Risk Management
- Undertake safeguarding enquiries and risk assessments.
- Support effective management of risk while maintaining a person-centered approach.
- Contribute to Mental Capacity Act (MCA) processes and safeguarding duties.
Urgent & Emergency Care
- Support discharge planning and work closely with health, voluntary and community partners.
- Contribute to Duty and TOCH assessments.
- Support DST/CHC processes where required.
- Liaise with system partners to coordinate appropriate support and services.
Practice Development
- Contribute to mentoring and practice development.
- Support less experienced staff and students where required.
- Work collaboratively within a multi-disciplinary environment.
Working Arrangements
- Hybrid working is available, with 2 days in the office and 3 days remotely, subject to service requirements.
- Visits will be carried out across the North of Nottinghamshire.
- The role will be based across Bassetlaw Hospital and Retford Hospital within the UEC North service.
- Working hours are Monday to Thursday 8:30am–5:00pm and Friday 8:30am–4:30pm.
