We are seeking an experienced Social Worker to join a dedicated Community Mental Health Team for Older Adults in Bracknell Forest. This is an excellent opportunity for a skilled practitioner with experience in adult social care, mental health, safeguarding, and strengths-based practice. What you will do

Manage a caseload of older adults requiring assessment, support, intervention, and ongoing review.

Apply a strengths-based approach to connect individuals with community resources, social networks, and support services that promote independence and wellbeing.

Undertake person-centered assessments and develop support plans that maximize independence, choice, and control while ensuring legal and cost-effective outcomes.

Work collaboratively with individuals, careers, families, advocates, and partner agencies to deliver holistic and coordinated support.

Undertake Mental Capacity Act assessments and act in the best interests of individuals who lack capacity, ensuring appropriate safeguards are in place.

Support the implementation and review of person-centered plans to ensure identified needs and outcomes are achieved.

Participate in safeguarding enquiries and act as an assessing officer in accordance with local safeguarding procedures.

Contribute to the team duty rota and respond appropriately to urgent situations and emerging risks.

Maintain accurate and timely case recordings, assessments, reports, and documentation in line with legislation, policy, and professional standards.

Take responsibility for professional development, actively engaging in supervision and reflective practice.

Essential Requirements

Qualified Social Worker with current Social Work England registration.

Minimum 2 years’ post-qualified Social Work experience.

Previous experience within Adult Social Care, Mental Health Services, or Community-Based Social Work Teams.

Knowledge and practical experience of the Mental Capacity Act 2005 and adult safeguarding procedures.

Strong assessment, care planning, risk management, and report-writing skills.

Ability to work effectively within a multi-disciplinary and multi-agency environment.

Commitment to strengths-based and person-centered practice.
