Job Description
Job Title: Social Worker – MH & LD&A
Specialism: Adult Services
Location: Surrey, UK
Salary: Up to £38.69 Per Hour
Type: Full Time / Temporary
We are seeking an experienced Social Worker to work as part of a multidisciplinary team providing high-quality social work support to adults with mental health issues, learning disabilities and autism.
This role involves assessing the needs of service users, ensuring care and support are well coordinated, and advocating for their rights and choices while working in accordance with relevant legislation and best practice guidelines.
What you will do
Assessment and Care Planning: Assess the needs of service users and ensure their care and support are effectively coordinated.
Person-Centered Practice: Apply person-centered approaches to ensure individuals’ needs, rights and choices remain central to practice.
Safeguarding: Apply safeguarding principles and procedures when working with vulnerable adults.
Social Work Practice: Provide high-quality social work support to individuals with learning disabilities, autism and mental health needs.
Advocacy: Advocate for the rights and choices of service users and promote appropriate support based on their individual needs.
Case Management: Use effective assessment and case management skills to support positive outcomes for service users.
Multidisciplinary Working: Work effectively as part of a multidisciplinary team while also managing responsibilities independently.
Requirements
- Recognized Social Work qualification, such as a BA or MA in Social Work.
- Social Work England (SWE) registration.
- Experience working with adults with mental health issues, ideally within a community or clinical setting.
- In-depth knowledge of the Care Act 2014 and other relevant legislation.
- Knowledge of safeguarding principles and procedures.
- Understanding of person-centered approaches and their practical application.
- Excellent communication, assessment and case management skills.
- Compassionate and resilient approach when working with vulnerable individuals.
- Ability to work independently and as part of a multidisciplinary team.
- Full UK driving license and access to a vehicle is required.
