We are seeking an experienced Social Worker to join a dedicated Mental Health Hospital Discharge Team based in Barnet. This is an excellent opportunity for a skilled practitioner to support adults with mental health needs through safe, effective, and person-centered hospital discharge planning.

Working across in-borough and out-of-borough psychiatric wards, you will play a key role in facilitating timely discharges, promoting independence, and ensuring individuals receive the right care and support to achieve positive outcomes within their communities.

What You Will Do

Hospital Discharge Planning: Support adults with mental health needs through assessment, discharge planning, and transition into community-based services.

Care Act Assessments: Undertake strengths-based assessments in accordance with the Care Act 2014 and relevant statutory frameworks.

Care and Support Planning: Develop personalized, outcomes-focused care and support plans that promote independence and wellbeing.

Mental Capacity Assessments: Undertake Mental Capacity Act assessments and apply relevant legislation to support informed decision-making.

Risk Assessment and Management: Identify, assess, and manage risks while promoting positive outcomes and safeguarding vulnerable adults.

Multi-Agency Working: Work collaboratively with NHS partners, psychiatric wards, community services, and other professionals to coordinate effective discharge arrangements.

Safeguarding Practice: Promote adult safeguarding principles and undertake safeguarding activities where required.

Case Management: Manage a complex caseload while maintaining accurate records, assessments, reports, and statutory documentation.

What We Are Looking For

  • Qualified Social Worker with current Social Work England registration.
  • Experience within Adult Social Care services.
  • Experience supporting hospital discharge pathways, ideally within mental health settings.
  • Strong knowledge of the Care Act 2014, Mental Capacity Act, and adult safeguarding legislation.
  • Experience completing assessments, care planning, and risk management activities.
  • Excellent communication, analytical, and report-writing skills.
  • Ability to work effectively within multidisciplinary and multi-agency environments.
  • Commitment to delivering person-centered and strengths-based practice.
