Job Title: Social Worker – Learning Disability Team

Type: Temporary / Full Time
We are seeking an experienced Social Worker to join a specialist Learning Disability Team based in Cambridgeshire. This is an excellent opportunity for a motivated practitioner with significant experience supporting adults with Learning Disabilities and Autism, alongside a strong background in reviewing complex care packages. Working as part of a county-wide project, you will collaborate with the Senior Social Worker, Contracts Team and Brokerage Team to deliver high-quality, person-centred services that promote independence and achieve positive outcomes.

What You Will Do
- Complete Care Act 2014 assessments, including complex and high-cost care package reviews.
- Undertake timely reviews following changes in individuals’ needs or circumstances.
- Carry out face-to-face assessments and reviews across Cambridgeshire.
- Review complex care packages to ensure they continue to meet assessed needs and provide value for public resources.
- Work collaboratively with the Senior Social Worker, Contracts Team and Brokerage Team to develop effective care solutions.
- Involve individuals, families, carers and advocates throughout the assessment and review process.
- Analyse care and support arrangements to promote independence, wellbeing and person-centred outcomes.
- Maintain accurate, timely and defensible case records in accordance with statutory requirements.
- Deliver a minimum of two completed and recorded assessments each week.
Essential Requirements
- Qualified Social Worker with current Social Work England registration.
- Significant experience working with adults with Learning Disabilities and Autism (LD&A).
- Experience reviewing complex and high-cost care packages.
- Strong knowledge of the Care Act 2014 and Mental Capacity Act.
- Experience undertaking project-based social work within Adult Services is highly desirable.
- Full UK driving licence with access to your own vehicle, as regular travel across Cambridgeshire is required.
