Job Description
Social Worker (2+ Years) – Learning Disabilities & Transitions – London Borough of Islington
The London Borough of Islington is seeking experienced Social Workers to join its Islington Learning Disability Partnership, supporting both adults and young people transitioning from children’s to adult services. We have multiple vacancies within the Adults Team and the Transitions Team, offering a rewarding opportunity to make a difference within a supportive multi-disciplinary setting.
Key Responsibilities:
- Complete Care Act Assessments, reviews, and support plans for adults with learning disabilities and young adults aged 18–25.
- Support individuals and their families through case management, statutory safeguarding work, and multi-agency coordination.
- Assist in reducing the backlog of reviews/assessments for adults with learning disabilities in a timely and effective manner.
- Work directly with young adults transitioning into adult services, promoting independence and person-centred outcomes.
- Ensure all statutory duties under the Care Act 2014 and safeguarding procedures are met.
Essential Requirements:
- Qualified Social Worker (QSW) registered with Social Work England.
- Minimum of 2 years’ post-qualified experience in adult social care or learning disabilities settings.
- Proven ability to manage caseloads of 20–25 cases efficiently and independently.
- Strong assessment, care planning, and safeguarding skills.
- Willingness to attend in-person visits and meetings (minimum two days per week in the office).
Additional Information:
- Location: 222 Upper Street, Islington (hybrid working model).
- Full-time: 35 hours per week.
- Inside IR35.
