Type: Temporary / Full Time
We are seeking an experienced Social Worker to join a Learning Disabilities Team within Adult Social Care in Somerset. This is an excellent opportunity for a skilled practitioner to work within a multi-disciplinary environment, supporting adults with learning disabilities and their carers through strengths-based, person-centred social work practice.
Working as part of a collaborative team, you will undertake assessments, manage complex cases, promote independence, and safeguard vulnerable adults while ensuring positive outcomes are achieved.

What You Will Do
Assessment and Care Planning: Undertake comprehensive assessments of need, risk, and eligibility, developing person-centred care and support plans.
Case Management: Manage a varied caseload, ensuring timely interventions, reviews, and effective support arrangements.
Safeguarding Practice: Assess and manage risks, ensuring vulnerable adults are protected from harm and receive appropriate support.
Learning Disabilities Support: Work with adults with learning disabilities to promote independence, wellbeing, and positive life outcomes.
Carers Assessments: Complete carers assessments and support planning in accordance with relevant legislation and guidance.
Multi-Agency Working: Develop effective working relationships with health services, housing providers, voluntary organisations, and other key partners.
Crisis Intervention: Respond appropriately to individuals presenting in crisis, applying relevant legislation, policy, and professional judgement.
Report Writing and Documentation: Prepare clear, analytical assessments, reports, and records to support decision-making and service provision.
Professional Practice: Contribute to service improvement, support colleagues and students where appropriate, and maintain high standards of professional practice.
What We’re Looking For
- Qualified Social Worker with current Social Work England registration.
- Experience working within Adult Social Care.
- Knowledge of relevant social care legislation, safeguarding responsibilities, and best practice.
- Experience working within multi-disciplinary and multi-agency environments.
- Strong assessment, care planning, and risk management skills.
- Full driving licence and access to a vehicle.
- Experience working with adults with learning disabilities.
- Experience supervising or supporting less experienced staff or students.
- Previous experience working alongside health professionals.
