We are seeking an experienced Social Worker to join the Hospital Team based at Harplands Hospital in Stoke-on-Trent. This is an excellent opportunity for a qualified social worker with experience in adult social care to support individuals through hospital discharge and ongoing care planning. Working within a collaborative multidisciplinary environment, you will play a key role in delivering person-centered interventions that promote positive outcomes.

What you will do

Hospital Social Work: Complete strengths-based assessments and provide effective social work interventions within a hospital setting.

Discharge Planning: Support safe and timely hospital discharges by working closely with health professionals, individuals, families, and careers.

Care Planning: Develop person-centered care and support plans that promote independence and wellbeing.

Multi-Agency Working: Collaborate with NHS colleagues and partner organizations to ensure coordinated and effective service delivery.

Case Management: Manage an allocated caseload while maintaining accurate records and meeting statutory responsibilities.

Safeguarding: Identify and respond appropriately to safeguarding concerns, ensuring adults receive the support and protection they require.
