Job Title: Social Worker – Assessment & Safeguarding Team

Type: Temporary / Full Time

We are seeking an experienced Social Worker to join the Assessment & Safeguarding Team in Haringey. This is an excellent opportunity for a skilled practitioner with post-qualification experience in Adult Social Care, strengths-based practice, safeguarding, and multi-agency working.

Working within a supportive and collaborative environment, you will manage a varied caseload while supporting adults to maximize their independence, improve their wellbeing, and achieve positive outcomes. You will work closely with residents, healthcare professionals, housing services, and community partners to deliver person-centered support and effective care planning.

What You Will Do

Strengths-Based Practice: Undertake strengths-based assessments, reviews, and support planning to promote independence, wellbeing, and resilience.

Case Management: Hold and manage a caseload, supporting adults and careers through effective assessment and intervention.

Information and Advice: Provide guidance on available services, community resources, assistive technology, aids and adaptations, benefits, employment, and volunteering opportunities.

Care Coordination: Assess and coordinate short-term care arrangements, including respite support where appropriate.

Safeguarding: Contribute to safeguarding enquiries, risk assessments, and protection planning while supporting Making Safeguarding Personal principles.

Multi-Agency Collaboration: Work closely with multi-disciplinary teams, health partners, housing services, and community organizations to achieve positive outcomes.

Record Keeping: Maintain accurate and timely electronic case records and prepare documentation required for panels and decision-making processes.

Continuing Healthcare: Support individuals through Continuing Healthcare pathways and ensure they are informed about appropriate service provision.

What You Will Bring

Social Work England registration.

A minimum of two years’ post-qualification experience within Adult Social Care.

Experience undertaking assessments, reviews, support planning, and safeguarding work.

Strong knowledge of strengths-based practice and person-centered approaches.

Excellent communication, organizational, and case management skills.

The ability to work effectively within a multi-disciplinary environment and build positive working relationships with residents, careers, and partner agencies.
