We are seeking an experienced Social Worker to join the Assessment and Intervention Team in Wrexham. This is an excellent opportunity to work within a fast-paced and supportive service, undertaking assessments and interventions that promote the safety, wellbeing, and positive outcomes of children, young people, and their families.
Working as part of a dedicated team, you will manage a varied caseload, complete high-quality assessments, and contribute to effective safeguarding and intervention planning while developing strong partnerships with families and professionals.

What You Will Do
Assessment and Intervention: Complete comprehensive assessments and interventions to identify needs, risks, and appropriate support arrangements.
Safeguarding Responsibilities: Undertake Section 47 enquiries and contribute to safeguarding processes to ensure the protection and wellbeing of children and young people.
Case Management: Manage an allocated caseload, delivering timely and effective social work interventions while escalating complex matters where required.
Care and Support Planning: Develop and review evidence-based care and support plans, ensuring identified needs are met and outcomes are achieved.
Risk Assessment and Management: Identify, assess, and manage risk, working collaboratively with children, families, carers, and partner agencies.
Multi-Agency Working: Build and maintain effective relationships with professionals and agencies to ensure coordinated and person-centred support.
Duty Responsibilities: Participate in the team duty rota, responding to urgent referrals, crisis situations, and safeguarding concerns as they arise.
Recording and Reporting: Maintain accurate records and prepare high-quality documentation in accordance with statutory requirements and professional standards.
