We are seeking an experienced Social Worker to join an Adults Transition Team based in Stoke-on-Trent. What you will do
Adult Social Care Practice: Undertake assessments and provide effective social work interventions to support adults with varying needs.
Transition Support: Work within the Transition Team to support individuals moving through key stages of care and service provision.
Person-Centered Assessments: Complete comprehensive assessments, develop support plans, and ensure individual needs and outcomes remain central to practice.
Multi-Agency Working: Collaborate with health professionals, partner agencies, families, and other stakeholders to deliver effective support.
Case Management: Manage an allocated caseload while maintaining accurate records, meeting statutory responsibilities, and delivering high standards of practice.
Safeguarding: Identify and respond appropriately to safeguarding concerns, ensuring adults are protected and supported.
