Job Description

Job Title: Senior Practitioner – Adults & Communities

Salary: Umbrella £29.77 per hour
Type: Temporary / Full Time

We are seeking an experienced Senior Practitioner Social Worker to join the Care Act Assessment Team in Hereford. This is an excellent opportunity for a qualified and experienced social worker with strong knowledge of adult social care, case management, assessment and support planning.

The successful candidate will support service users across Herefordshire who have recently left hospital through Discharge to Assess and may require ongoing services, information, advice and signposting. You will work within a fast-paced environment, supporting timely assessments and achieving positive outcomes for service users.

What You Will Do

Assessment and Support Planning: Carry out short-term assessments and support planning for service users throughout Herefordshire.

Discharge to Assess: Support service users who have recently left hospital through the Discharge to Assess process, identifying ongoing support requirements.

Case Management: Apply strong case management skills and effective decision-making throughout the assessment and review process.

Supervision and Support: Support staff on a day-to-day basis and undertake formal and informal supervision as required.

Team Responsibilities: Take responsibility for team duty, allocations, triaging and sign-offs of assessments and workflows.

Risk Management: Apply positive risk-taking and effective conflict resolution skills to support safe and effective outcomes.

Adult Social Care Practice: Apply a strong knowledge of social care practice and legislation, ensuring responses are person-centered, strengths-based, safe and effective.

Specialist Practice: Contribute knowledge and experience across areas including hospital discharge, Continuing Health Care, Mental Capacity Act assessments, mental health, learning disability, physical disability and older adults.

Partnership Working: Provide appropriate information, advice and signposting to support service users to access the right services and support.

Independent Working: Work on your own initiative to meet the requirements of the assessment and review process.
