Type: Ongoing / Full Time

We are seeking an experienced Senior Practitioner to join the Access & Duty Team based in Hackney, London. This is an excellent opportunity for an experienced Adult Social Care professional with strong leadership and supervisory experience. You will support the delivery of high-quality, strengths-based practice, provide guidance and supervision to staff, manage complex cases, and work collaboratively with multidisciplinary partners to achieve positive outcomes for adults with complex needs.

What You Will Do

  • Provide professional supervision, guidance, and support to team members, promoting high standards of practice and continuous improvement.
  • Manage and oversee complex assessments, ensuring interventions are strengths-based, outcome-focused, and compliant with relevant legislation.
  • Support safeguarding activity, including leading safeguarding enquiries, chairing meetings where appropriate, and ensuring statutory responsibilities are met.
  • Allocate work, monitor performance, and support staff development through regular supervision and quality assurance.
  • Work collaboratively with multidisciplinary teams, health partners, and community organisations to deliver integrated services.
  • Prepare high-quality reports, maintain accurate case records, and ensure compliance with departmental standards and legislative requirements.
  • Promote personalised support, enabling adults and carers to make informed choices and maximise independence.
  • Contribute to service development, quality assurance activities, and continuous improvement initiatives across Adult Social Care.
