Job Description

Job Title: SEND Assessment & Review Officer

Type: Temporary / Full Time

We are seeking experienced SEND Assessment & Review Officers to join a dedicated SEND service in Somerset. This is an excellent opportunity to support the delivery of high-quality Education, Health and Care (EHC) processes while helping to manage a backlog of phase transfers and draft plans.

Working within a supportive and collaborative environment, you will take ownership of a caseload of children and young people, ensuring that statutory processes are completed efficiently and that positive outcomes remain at the center of decision-making.

What You Will Do

Case Management: Take ownership and accountability for an allocated caseload of children and young people throughout the SEND process.

EHC Plan Management: Draft and amend Education, Health and Care Plans and associated documentation in line with statutory requirements.

Phase Transfer Work: Support the completion of phase transfers, ensuring timescales are met and plans are progressed effectively.

Stakeholder Engagement: Maintain regular communication with schools, educational settings, families, and partner agencies.

Consultation Processes: Undertake consultations with educational settings and other relevant professionals to inform planning and decision-making.

Statutory Compliance: Ensure all SEND processes are completed accurately, efficiently, and within statutory deadlines.

Record Keeping: Maintain clear, accurate, and up-to-date records in line with organizational policies and procedures.
