Job Description

Job Title: Qualified Social Worker – Adults with Learning Disabilities & Autism Team

Salary: £28.40 Per Hour (Umbrella)
Type: Temporary / Full Time

We are seeking an experienced Qualified Social Worker to join an Adults with Learning Disabilities and Autism Team based in Telford. This role focuses on completing Care Act reviews, strengths-based assessments, support planning, and safeguarding interventions to promote independence, wellbeing, and positive outcomes for adults.

This is an excellent opportunity for a skilled practitioner with strong knowledge of Adult Social Care legislation, including the Care Act 2014 and Mental Capacity Act 2005. You will work collaboratively with individuals, families, and partner agencies while contributing to a supportive team environment focused on high-quality practice.

What You Will Do

Adult Social Care Practice: Manage a professional caseload, completing assessments, support plans, reviews, and risk assessments to support adults in achieving positive outcomes.

Care Act Reviews: Complete Care Act reviews for adults with Learning Disabilities and Autism, ensuring individual needs, strengths, and aspirations remain central to practice.

Safeguarding: Promote safeguarding practice and undertake appropriate safeguarding interventions and investigations where required.

Strengths-Based Assessments: Apply strengths-based and asset-based approaches to empower individuals and promote independence.

Multi-Agency Working: Collaborate with statutory, voluntary, and community partners to achieve agreed outcomes and provide coordinated support.

Case Management: Maintain accurate case records, reports, and documentation while effectively managing workload and meeting professional standards.

Professional Contribution: Attend peer reviews, reflective practice sessions, and team discussions while providing professional advice and guidance where required.

Additional Responsibilities: Undertake duty responsibilities as part of the team rota and support continuous improvement within the service.

Requirements

  • Qualified Social Worker registered with Social Work England.
  • Knowledge and understanding of Adult Social Care legislation and best practice, including the Care Act 2014 and Mental Capacity Act 2005.
  • Ability to complete evidence-based assessments, make informed decisions, and manage risk effectively.
  • Strong communication and partnership-working skills.
  • Commitment to continuous professional development and reflective practice.
  • Driving Licence required.
