Type: Ongoing / Full Time
We are seeking an experienced Social Worker to join the Learning Disability Team within Adult Services in Rhondda Cynon Taf. This is an excellent opportunity for a skilled practitioner to undertake reviews and assessments, supporting adults with learning disabilities to achieve positive outcomes, maintain independence, and access appropriate care and support services.
Working within a strengths-based and outcome-focused framework, you will collaborate with individuals, families, and partner agencies to ensure high-quality social work practice and person-centered support.

What You Will Do
Assessment and Review Work: Undertake strengths-based assessments and reviews in accordance with the Social Services and Well-being (Wales) Act 2014.
Care and Support Planning: Develop, implement, and review care and support plans that promote independence, choice, and wellbeing.
Risk Assessment: Identify, assess, and manage risks while supporting individuals to achieve positive outcomes.
Direct Engagement: Work directly with adults, families to understand needs, strengths, and desired outcomes.
Multi-Agency Working: Collaborate with health professionals, service providers, and partner agencies to coordinate effective support.
Safeguarding Practice: Promote the safety and wellbeing of vulnerable adults and contribute to safeguarding processes where required.
Resource Coordination: Liaise with service providers and support services to secure appropriate resources that meet assessed needs.
Record Keeping: Maintain accurate and timely assessments, reviews, care plans, and case recordings in line with statutory and organizational requirements.
Professional Contribution: Participate in team meetings, peer sessions, supervision, and service development activities to support continuous improvement.
