Job Title: Adult Social Worker – Young Adults (Transitions) Team

Type: Temporary / Full Time

We are seeking an experienced Adult Social Worker to join the Young Adults (Transitions) Team in Herefordshire. This is an excellent opportunity to support young people aged 18–25 as they transition from children’s services into adult social care. Working within a strengths-based framework and in line with the Care Act 2014, you will play a key role in promoting independence, wellbeing, and positive outcomes for young adults.

What You Will Do

Strengths-Based Assessments: Complete comprehensive assessments in line with the Care Act 2014, identifying eligible needs and desired outcomes.

Care and Support Planning: Develop and review care and support plans that promote independence, wellbeing, and positive long-term outcomes.

Safeguarding Responsibilities: Undertake Section 42 safeguarding enquiries, ensuring appropriate action is taken to protect adults at risk.

Mental Capacity and Best Interests: Complete Mental Capacity Act assessments and contribute to best-interest decision-making processes where required.

Continuing Healthcare: Participate in Continuing Healthcare (CHC) assessments and multi-agency discussions.

Court Work: Contribute to court-related work and provide high-quality reports and documentation where necessary.

Multi-Agency Partnership Working: Work closely with young adults, families, health professionals, education providers, and partner agencies to support safe and effective transitions into adult services.
