Social Worker – Community Team for People with Learning Disabilities (CTPLD) Team
Specialism: Adults Services
Location: Bracknell, UK
Salary: £29.74 Per Hour PAYE / £38.00 Per Hour Ltd AWR
Type: Temporary / Full Time
We are seeking an experienced Social Worker to join the CTPLD Team in Bracknell. This is an excellent opportunity for a qualified practitioner with experience in adult social care, learning disabilities and autism.
You will work alongside individuals, advocates and careers to develop person-centered plans that promote independence, choice and control. You will also contribute to safeguarding, assessments, care and support planning, and multi-disciplinary working.
What You Will Do
Assessment and Care Planning: Conduct assessments, develop care and support plans, and review and monitor those plans in line with Care Act responsibilities and other statutory frameworks.
Strength-Based Practice: Apply a strength-based approach to connect people with their communities and social networks, helping to identify solutions and promote independence.
Person-Centered Support: Work with individuals, advocates and careers to develop person-centered plans that meet identified needs while maximizing independence, choice and control.
Career Support: Work with family careers to identify their needs and develop appropriate plans to support them.
Resource Allocation: Complete the Resource Allocation System when required and present support plans for approval in accordance with the relevant Scheme of Delegations.
Safeguarding: Participate in safeguarding assessments and planning to support the safety and wellbeing of vulnerable adults.
Multi-Disciplinary Working: Participate in multi-disciplinary meetings and work collaboratively with individuals, families and professionals.
Mental Capacity: Act in the best interests of individuals who lack mental capacity, applying the principles of the Mental Capacity Act 2005.
Duty Arrangements: Participate in the team’s duty arrangements and ensure departmental policies and procedures are followed.
Requirements
- Recognized Social Work qualification and registration with Social Work England.
- Experience working with individuals with learning disabilities and autism.
- Considerable experience in a social care or healthcare setting.
- Knowledge of relevant legislative frameworks and national policies.
- Experience completing mental capacity assessments.
- Understanding of safeguarding vulnerable adults and promoting their welfare.
- Strong report-writing and communication skills.
- Ability to priorities and organize workload effectively.
- Good IT skills, including Microsoft Word, Excel and Outlook.
- Person-centered approach with empathy and respect for diversity.
- Ability to work effectively both independently and as part of a team.
- Flexibility and the ability to take responsibility and accountability at the appropriate level.
- Full UK driving license or valid equivalent.
