Job Description
Job Title: Social Worker – Safeguarding Triage and Urgent Response
Specialism: Adult Social Care
Location: Bath, UK
Salary: £35.00 Per Hour PAYE
Type: Temporary
We are seeking an experienced Social Worker to join a Safeguarding Triage and Urgent Response Team within Adult Social Care in Bath. This is an excellent opportunity for a skilled practitioner to provide strengths-based, person-centered support to adults and careers, responding to safeguarding concerns and urgent care and support needs.
Working within a supportive multidisciplinary team, you will help establish individuals’ preferred outcomes, assess risks and support timely safeguarding decisions while promoting independence, wellbeing, prevention and access to community resources.
What you will do
Safeguarding: Work with individuals to establish their preferred outcomes following safeguarding concerns and gather relevant information to support appropriate decision-making.
Safeguarding Referrals: Complete assessments and make recommendations regarding the outcome of safeguarding referrals.
Care Act Assessments: Undertake strengths-based Care Act assessments with adults and carers when required.
Duty Service: Undertake office-based duty once a week, responding to calls from members of the public and professionals regarding concerns about abuse or neglect.
Case Management: Manage an allocated caseload, ensuring timely progression and effective flow through the system.
Case Recording: Maintain high-quality case records, safeguarding documentation and reports.
Multi-Agency Working: Work collaboratively with health colleagues, families, housing and voluntary sector partners to support positive outcomes.
Early Intervention: Promote early intervention, prevention and access to appropriate community resources.
Strengths-Based Practice: Deliver high-quality, strengths-based social work focused on reducing ongoing risks and supporting individuals to achieve their preferred outcomes.
