We are seeking a skilled and committed Social Worker to join a specialist Children with Disabilities Team in Herefordshire. What You Will Do

Holistic Assessments: Complete holistic assessments to understand the individual needs of children and young people with disabilities.

Support Planning: Plan and coordinate appropriate services and support to help children and young people achieve positive outcomes.

Family Engagement: Build trusting and positive relationships with children, young people and their families, ensuring their voices and experiences remain central to practice.

Multi-Agency Working: Work collaboratively with professionals across health, education and social care to coordinate effective support.

Promoting Independence: Support children and young people to thrive, reach their full potential and develop greater independence.

Safeguarding and Wellbeing: Ensure the safety, wellbeing and participation of children and young people remain central to all aspects of practice.

Restorative Practice: Contribute to a collaborative and reflective approach to social work within a supportive specialist team.

Essential Requirements

• Passionate about supporting children and young people with disabilities and their families to achieve the best possible outcomes.
• Skilled in assessment, planning and coordinating multi-agency support.
• Committed to working collaboratively, inclusively and creatively with children, families and professionals.
• Resilient, reflective and confident when managing complex situations and making child-centered decisions.
• Able to work effectively within a supportive and specialist team environment.
• Registered with Social Work England and hold a recognized Social Work qualification such as a Degree, DipSW or CQSW.
